Correctly filling out a paper form of temporary disability is a responsibility that lies not only with the medical institution, but also with the employee’s employer. What pen should I use to fill out a sick leave form?
Paragraph 65 of the order (Order No. 624n) states that only the following types of pens are allowed to be used to fill out sick leave:
- gel;
- feather;
- capillary.
It is important to note the following point: correctly filling out a sick leave certificate in 2020 does not require the use of a ballpoint pen for these purposes.
The FSS Letter No. 17-03-09/06-3841P dated October 23, 2014 states that combined filling is also possible. That is, some of the data is entered by hand, and the other on the computer. Using only the capabilities of computer technology for these purposes is another option that does not contradict existing rules.
The organization name is incorrect: how to correct the error?
Usually every employee knows the abbreviated name of the company in which he works. But the exact and full name is indicated in the constituent documents, and it may differ significantly from the usual name.
This is the main reason for the doctor to fill out the employer field incorrectly.
If the name of the organization is incorrectly indicated on the sick leave, it is considered invalid.
Some companies accept such certificates of temporary disability, but in the future this may lead to problems with the calculation of benefits from the Social Insurance Fund if it is not possible to identify the company by the specified name. HR department employees are advised to carefully read the contents of the sick leave certificate before accepting it.
If there are errors in the name of the company, then the employee needs to return the document, provide the correct name of the organization and request to receive a duplicate certificate of incapacity for work, properly executed. The doctor must fill out a new sick leave form.
It is prohibited to correct an error using a proofreader, crossing out, or a blade. If the employer sent a certificate of temporary disability to the Social Insurance Fund, which incorrectly indicated the name of the company, and received a refusal to pay, then he has the right to appeal such a decision.
Judicial practice shows that entrepreneurs usually manage to obtain sick leave benefits for their employees from the Social Insurance Fund, despite errors in the documents provided.
Sick leave: filling out by the employer and sample 2020
What data does the employer’s employee to whom this authority is delegated write down on the temporary disability certificate?

Next, indicate the subordination code and registration number. Below is information about the employee: TIN and SNILS. Then they indicate the length of service the subordinate has developed, as well as the dates during which he was unwell.

What to do if the place of work is not indicated?
Information about the place of work must be filled out by the doctor issuing the bulletin. But some doctors deliberately leave this field blank because they do not know how to correctly spell the name of the organization in which the patient works.
There are often situations when a citizen who has applied for a certificate of incapacity for work due to illness does not know the exact name of the company, since he recently got a job in it.
When an employee brings a certificate of incapacity for work, which does not indicate the name of the place of work, then the head of the enterprise has a question about what to do. Some company directors fill out this field themselves. This is possible if a sick leave certificate is provided on paper.
But you need to understand that the organization has the right to refuse to accept a certificate without indicating the employer. Cases when a doctor leaves a blank line about his patient’s place of work occur quite often. Therefore, the question, if the name of the organization is incorrectly indicated on the sick leave, is relevant.
The company management should not fill out the field dedicated to the name of the employer instead of the doctor. This is illegal.
In addition, if we are talking about an electronic certificate of temporary disability, then the employer does not have the right or opportunity to make adjustments to the block “To be completed by a doctor of a medical institution.” It is acceptable to accept such sick leave from an employee.
The absence of a company name in the bulletin does not deprive the subordinate of the right to receive benefits, but problems may arise with the Social Insurance Fund (SIF). Therefore, a more correct solution would be to invite the employee to contact the medical institution with an incorrectly completed sick leave certificate to the doctor who issued it, with a request to fill in the missing information.
An employee should not independently fill out the column dedicated to the name of the employer . This may result in the document being invalidated. Entering information into the form is carried out taking into account a number of rules, using certain types of pens.
The employer or employee has the right not to accept such a document and demand the issuance of a correctly executed certificate of temporary disability.
If not all fields on the sick leave sheet are filled in or there are any errors in the text, then it is considered invalid.
FSS pilot project: filling out sick leave
The question of filling out a temporary disability certificate under the Social Insurance Fund pilot project adopted in some regions worries many personnel specialists and accountants. However, it should be noted that the pilot project does not affect the rules for registering sick leave at all.
Its essence is that the amount of money due to the employee from the fourth day of his period of incapacity for work is paid by the Social Insurance Fund. The sample for filling out sick leave by the employer remains the same.

How is sick leave calculated?
To calculate sick leave, you need to know your earnings for the previous two calendar years. Only official income on which all taxes have been paid is taken into account. The assignment of benefits directly depends on the employee’s insurance coverage. For the first three days of illness, benefits are paid at the expense of the employer, and starting from the fourth - at the expense of the Social Insurance Fund. The amount of benefits for a certificate of incapacity for work has no fixed value and depends mainly on the employee’s earnings and length of service:
- with 8 or more years of experience, the benefit amount will be 100% of average earnings;
- if the experience is from 5 to 8 years - 80% of average earnings;
- for less than 5 years of experience, the benefit is 60% of average earnings.
The entire procedure for calculating sick leave can be conditionally divided into several successive stages. We suggest looking at it with an example. So, let’s imagine that we are on sick leave for 8 days and our salary is 30 thousand rubles per month. Therefore, we will calculate the total amount of income for 2020 and 2020: 30,000 rubles. x 24 months = 720,000 rub. Next, we display the average salary per day. For this purpose 720,000 rubles. / 730 and we get an average daily income of 986.30 rubles.

How to fill out the new form correctly?
It is important to fill out the temporary disability form correctly, taking into account a number of rules. Making a mistake can lead to the document being declared invalid and the Social Insurance Fund refusing to pay for the employee’s sick days.
General rules for filling out the form:
- write in block letters;
- use Arabic numerals;
- do not go beyond the boundaries of the cell;
- each cell is reserved for one letter or number;
- do not indicate a direct diagnosis on the form;
- Do not cross out or paint over the text with a proofreader. It is also prohibited to use a blade or eraser to correct an error.
The rules and procedure for filling out sick leave are slightly different for representatives of medical institutions and the employer. Each organization has its own instructions.
Instructions for medical institutions
The first part of the ballot is filled out by a representative of the medical institution. In this case, you should be guided by paragraph 56 of the Rules for issuing such documents.
The doctor should adhere to the following instructions:
- At the top of the form in the first line it is indicated whether the primary sick leave or its duplicate is issued. In the latter case, the number of the lost form is placed opposite. If a person falls ill for a long period, then he is issued several certificates of incapacity for work. Each subsequent document is marked with the number of the previous one;
- write the name of the medical institution and its exact address. Between words and numbers you need to leave one empty cell as a space. If all the information does not fit in the allotted lines, then it is permissible to shorten it. The address may be specified in any order;
- The date of issue of the form and the state registration number of the organization are indicated. OGRN is noted if the company name has been abbreviated;
- if sick leave was issued due to the need to care for a temporarily disabled family member, then the corresponding field is filled in. The full name, age of the person in need of help, and relationship with him/her are indicated;
- information about the patient is entered: date of birth, first name, patronymic and last name, SNILS, Taxpayer Identification Number, gender, cause of disability, type of place of work (part-time or main);
- The table “Exemption from work” is filled out. It indicates the start and end dates of the sick leave, the name and position of the doctor, and his signature. When writing a doctor's specialization or the name of a medical institution in this table, it is permissible to use abbreviations;
- a date is set from which the patient can begin work. In case of extension of sick leave, the corresponding code is indicated in the “Other” column;
- At the bottom of the section, the doctor puts his signature and the seal of the medical organization;
- The spine of the temporary disability form is filled out. It contains minimal information from the previous sections. The medical history number, the date of issue of the certificate and the patient’s signature are added.
Instructions for employers
Before filling out a sick leave certificate, the employer should familiarize itself with a number of rules. They are given in letters of the FSSR No. 14-03-11/15-8605 dated September 14, 2011, No. 14-03-18/15-12956 dated October 28, 2011, No. 14-03-11/15-16055 dated December 23, 2011.
The employer fills out the certificate of incapacity for work in accordance with the following algorithm:
- information about the organization is indicated;
- it is noted that the employee works at the main place of work or part-time;
- FSS registration number and subordination code are entered. This data allows the company to be identified by employees of the Social Insurance Fund;
- SNILS and INN are indicated. Provides an opportunity to establish the employee’s identity;
- information about work experience is provided;
- if an employee was injured at work, then column “H-1” is filled in;
- the start date of work is indicated;
- information on the employee's salary is entered. The data is indicated excluding personal income tax. The company's accountant calculates the average daily and average monthly earnings. It is noted which part of the sickness benefit is paid by the employer and which by the Social Insurance Fund;
- The seal and signature of the head of the company are affixed.
